Service Authorization Form

By signing below, Customer acknowledges that the installation of Surf Air Wireless' equipment necessary for the Service may require the drilling of holes and minor cosmetic or other alterations to the site on which said equipment shall be installed.  Customer hereby authorizes Surf Air Wireless, its employees, sub-contractors, and agents to perform all work necessary for the proper installation of the equipment required for the Service and shall assume and/or incur the risk for all damage, alteration and/or modification as a result thereof (except damage caused by Surf Air Wireless’, its employees', sub-contractors', and agents' gross negligence), and shall indemnify and hold Surf Air Wireless, its employees, sub-contractors, and agents harmless therefore.  If the Customer is a new subscriber to a Surf Air Wireless Service, then this Authorization Form shall include, and hereby incorporates, by reference, all of the terms and conditions of the Terms of Service, Acceptable Use Policy and Privacy Statement posted on Surf Air Wireless' Website located at:

All Customer Equipment (as defined in the Terms of Service Agreement) shall remain the sole property of Surf Air Wireless.  By signing below, you hereby agree that Surf Air Wireless may pick up / retrieve its equipment at any time after cancellation of the account.  Surf Air Wireless reserves the right to cancel according to the Terms of Service Agreement or any violation of the Acceptable Use Policy.

911 Disclosure Notice and Acknowledgement

Surf Air Wireless 911 service is different from traditional wireline 911 and cellular/wireless 911, but it is a safe and reliable means of emergency dialing.

Surf Air Wireless customers have access to Enhanced 911. With E911 service, when you dial 911 your Surf Air Wireless phone number and registered address is sent to the local emergency center serving your location, and emergency operators have access to this information in order to send help and call you back if necessary.

We require you to provide the physical address where you will be using our service for 911 purposes. For E911, this address is used to route 911 calls to your local emergency center. When you initially provide your physical address, it can take up to 120 minutes to verify your address and activate 911 service at this address.

If you move this device to another location, you must update your address. You may register only one location at time. If you do not update your location, your 911 calls may be sent to an emergency center near your old address. When you change your location, you will need to contact Surf Air Wireless to update your address.

PLEASE NOTE THAT OUR 911 DIALING SERVICE WILL NOT FUNCTION IN THE EVENT OF A BROADBAND OR POWER OUTAGE OR IF YOUR BROADBAND INTERNET SERVICE PROVIDER (ISP), OR SURF AIR WIRELESS PHONE SERVICE IS TERMINATED.
